COVID-19 TEST CONSENT, DISCLAIMER AND WAIVER NOTICE
By booking and confirming an appointment for a Covid -19 Lab Test via the MDLink Platform, you hereby voluntarily consent to the Antigen and PCR Testing offered by MDLink (the “Testing”) as well as the following disclaimers:
- The Testing alone may not be sufficient to detect or rule out the possibility that you have been exposed to or are infected with COVID-19. You should carefully monitor your own symptoms and, notwithstanding the results of any Testing, you must stay home and consult with your physician if you experience symptoms of COVID-19.
- A negative result does not rule out COVID-19. False negative results may occur in patients who have indicated or whose clinical status or history would indicate they are currently taking high doses of biotin (> 10 mg per day). Biotin levels of 2.5 µg/mL have been demonstrated to result in false negative test results. False-negative results may also occur if the concentration of the target antigen in the clinical specimen is below the detection limits of the device used for the Testing.
- A positive result may be a false positive due to the fact that it picks up other strains of the coronavirus that you are immune to that can cause common cold symptoms.
- MDLink may be required by law to share the results of your test with public health authorities or any regulatory or governmental body.
- You agree to indemnify, release and waive any claim that may arise against MDLink, its agents, employees, officers, directors, subsidiaries, affiliates, licensors, and suppliers for any injury, risks, side effects, complications, damage, loss or expense resulting from the Testing.
